Responsibilities

Provide direct patient care under the supervision of the nursing team
Monitor residents’ health status
Administer medications and treatments with accuracy
Perform routine wound care
Assist with ADLs to restore or maintain independence
Document care plans
Report changes promptly
Collaborate with RNs, physicians, and therapists to coordinate comprehensive care
Participate in rounds
Assist with admissions and discharges
Ensure safety protocols are consistently followed to minimize risk and enhance comfort
Careful vitals monitoring
Infection control practices
