About The Position

Licensed Practical Nurse (LPN) needed for a travel assignment in Long Term Care, located in Geneva, New York. This assignment is in a Skilled Nursing Facility setting and is managed as a travel - temporary assignment with Healthforce Healthcare LLC. The contract length is 22 weeks, with a 5x8 rotating shift. Weekly pay is estimated between $1,266–$1,340 per week.

Requirements

Licensed Practical Nurse (LPN) license, active in New York state.
BLS certification (AHA or RQI required).
Minimum 1 year of LPN experience, preferably in a long-term care setting.
Ability to provide care to adult and geriatric residents and support developmentally appropriate care plans.
Demonstrated ability to work under the supervision of a Registered Nurse within a skilled nursing environment.

Responsibilities

Collect data on residents' physical and psychosocial condition and communicate findings to the RN and Interdisciplinary Team.
Contribute to a developmentally appropriate, culturally relevant plan of resident care reflecting nursing diagnoses and interventions, involving the resident, family, and healthcare team.
Recommend revisions to the plan of care after evaluating resident outcomes; identify referrals to ensure all aspects of care are addressed.
Provide direct and indirect skilled nursing care for assigned residents under the supervision of a Registered Nurse; follow facility policies and procedures.
Maintain current knowledge and competency through continuing education and inservice programs; participate in mentoring or orientation of staff; provide leadership within the professional practice setting.
Maintain safety and infection control practices; handle tasks with potential exposure to blood, body fluids, or tissues in accordance with policy.
