The Telehealth Triage Nurse (RN) triages or manages symptom-based patient encounters by phone or video using evidence-based triage protocols. The role helps reduce unnecessary visits to physicians, advanced practice providers, and emergency departments while providing self-care information. The nurse uses effective communication and information-gathering skills to determine appropriate patient disposition, serving patients across a range of needs, from healthy to acutely or chronically ill. Telehealth triage nursing follows the six-step nursing process: assessment, diagnosis, identification of goals and outcomes, planning, implementation, and evaluation. Common interventions include identifying and clarifying patient needs, providing health education, promoting patient advocacy and self-efficacy, and helping patients navigate the health care system.
