As a Supportive Oncology APP you will collaborate closely with Oncology, Hospice, and Palliative Care physicians and advanced practice providers to deliver patient-centered care focused on goals-of-care discussions, symptom management, and care coordination. Key responsibilities include conducting comprehensive patient assessments, performing physical examinations, and helping patients and families navigate complex medical decisions and transitions of care. You will play an integral role in ensuring a seamless, compassionate, and supportive experience throughout the patient's healthcare journey while promoting quality of life and aligning care with each patient's values and preferences. You will practice primarily at McKay Dee Cancer Center with occasional coverage in Layton.
