About The Position

The Supportive Housing Case Manager for Set-Aside Vouchers is an experienced social services professional responsible for providing high-quality, field-based, direct client services to families and individuals experiencing homelessness, helping participants obtain and maintain permanent housing using a Section 8 Voucher. The selected candidate will focus on stabilizing clients upon program entry, ensuring their needs are identified and met as appropriate (e.g. education and employment resources, primary physical and mental health, life skills, connection to maintain support services, etc.), ongoing work with clients to obtain and maintain permanent and stable housing, and support with landlord-tenant relations. The Supportive Housing Case Manager is dedicated to supporting and empowering participants through a trauma-informed lens with the overall goal of maximizing housing stability.

Requirements

  • A bachelor’s degree, preferably in social work, or in another behavioral science.
  • At least 2 years of case management experience is preferred.
  • Experience working with families in crisis and at-risk populations.
  • Experience with housing and case management techniques.
  • Experience with trauma-informed care.
  • Experience with housing first.
  • Experience with motivational interviewing.
  • Experience with crisis intervention techniques.
  • Experience with harm reduction strategies.
  • Experience with critical time intervention.
  • Highly motivated self-starter with the ability to coordinate projects/tasks simultaneously.

Responsibilities

  • Provide field based, case management services focused on independent living skills, housing stabilization, budgeting and financial literacy, community integration, problem solving, developing coping skills, etc.
  • Offer guidance and support throughout the housing search process, application and lease signing, move-in, and housing stabilization.
  • Provide strengths-based, client-driven case management that facilitates individualized service plans addressing short-term and long-term goals.
  • Utilize evidence-based practices in service delivery such as Trauma-Informed Care, Housing First, Motivational Interviewing, Harm Reduction, and Critical Time Intervention.
  • Work collaboratively with Opportunity Home San Antonio (formerly known as the San Antonio Housing Authority/SAHA) to provide Section 8 Vouchers to eligible families and/or single individuals.
  • Receive referrals from Coordinated Entry and screen potential clients for Set Aside Homeless Voucher program eligibility.
  • Accompany clients to voucher eligibility appointment at Opportunity Home San Antonio and assist clients through eligibility process.
  • Work collaboratively with clients to develop and update housing stability plans, based on assessments conducted, to assist participants to obtain or maintain permanent housing.
  • Utilize a variety of interventions to assist participants with overcoming barriers to housing stability, including increasing income and benefits.
  • Maintain on-going case load of participants in program by meeting with each participant on a monthly basis and maintaining bi-weekly contact with participants.
  • Maintain necessary documentation, reports and statistics in the Homeless Management Information System (HMIS) database and enter all data within timeframes as set forth by the HMIS Data Quality Plan.
  • Submit quarterly reports to Opportunity Home San Antonio on status of vouchers and supportive services provided.
  • Establish goals, priorities and timelines on a regular basis, ensuring timely and efficient management and completion of all tasks.
  • Communicate challenges with the Permanent Supportive Housing Program Manager and/or the Vice President of Supportive Housing.
  • Attend staff meetings and participate in organization-wide planning activities.
  • Handle other tasks assigned by the Permanent Supportive Housing Program Manager and/or the Vice President of Supportive Housing.
  • Keep up to date on compliance best practices and applicable government legislation by reading industry publications, attending workshops, seminars, and conferences.
  • Undertake all activities with the highest ethical standards.
