Case Manager, Permanent Supportive Housing

St Vincent De Paul Society District Council Of Marin•San Rafael, CA
•$31 - $33•Onsite

About The Position

The Case Manager, Permanent Supportive Housing (PSH) supports chronically homeless individuals, including people currently or formerly living in encampments, through intensive, ongoing, client-centered case management. The role helps clients reduce the effects of homelessness and work toward sustainable housing, while connecting them with mental-health and other services whether they are housed or not. The position calls for an energetic, self-starting professional who is passionate about supporting people experiencing homelessness, communicates effectively, pays close attention to detail, and is persistent in pursuing outcomes. Familiarity and comfort with street culture and community resources, and the ability to build trusting, hopeful relationships using a trauma-informed approach, are important. These are full-time, non-exempt positions reporting to the Program Manager, PSH. The work is in person and field-based, with regular work outside the office. Services may take place at SVdP offices, client residences, hospitals, housing sites, social-service and healthcare facilities, and other community locations. A valid California driver’s license is required to travel to these locations and provide in-person services. After a successful six-month performance review, one remote work day may be approved. Three positions are open: one requires Spanish fluency, and one is temporary through February 2027 to cover a leave of absence. St. Vincent de Paul Society of Marin County (SVdP) is an independent nonprofit social-services organization serving Marin County residents with food, housing, and crisis assistance. Its programs support more than 8,000 people annually and include a free dining room, homeless outreach, and housing and crisis assistance. SVdP is affiliated with the Society of St. Vincent de Paul and is governed by its own Board of Directors; donations received serve the people of Marin.
