The Supportive Housing Case Manager works with a caseload of 10–20 single-person and family households in supportive housing. Clients may experience mental health diagnoses, substance use, physical or developmental disabilities, child protection involvement, limited income, domestic violence, or chronic or long-term homelessness. The role supports a housing-first model using a trauma-informed approach to help clients maintain housing and work toward long-term self-sufficiency. The Case Manager conducts needs assessments, develops service plans with clients, coordinates services and resources, provides crisis support, and guides clients toward stability and successful completion of the housing program. The position also includes maintaining client records and required documentation, participating in an on-call rotation, collaborating with agency and partner staff, and transporting clients in Shelter House vehicles.
