About The Position

The Supervisor Care Manager is responsible for the quality of services to clients and the performance of the Care Managers. The position coaches Care Managers in order to meet the needs of clients through comprehensive care management. This position is also responsible for the efficiency of the day to day operations of the program including scheduling staff, hiring, training, regulatory compliance, and the safety of individuals, family members, and staff.

Requirements

  • Bachelor’s degree in a related field required.
  • Minimum 3 years of experience in service coordination.
  • Adult Health Homes (Health Home Plus Members): Licensed healthcare professional with prior experience in a behavioral health setting; OR Master’s degree with 2 years of supervisory experience in a behavioral health setting.
  • Must receive CANS (Child and Adolescent Needs and Strengths Assessment) certification score of 80 within 3 months of hire and annually thereafter.
  • Unrestricted, valid NYS driver’s license for minimum of 1 year with a clean driving record and minimum insurance coverage that meets agency standards.
  • Strong understanding of community services and related regulations on access and availability, as well as necessary resources.
  • Ability to multitask in a complex and fast pace environment and to work through a series of issues using independent judgment.
  • Develop, maintain and utilize a thorough knowledge of the legal and social rights of children, youth, adults and parents to assure that these rights are understood and protected as needed.
  • Knowledge of services provided both by the Agency and by external service providers, and ability to maintain contacts as needed in order to efficiently access care for children/youth/adults/families
  • Advocate for children/youth/families in order to achieve the best possible child/youth/adult/family outcomes; communicate reasonable expectations and facilitate the achievement of results.

Responsibilities

  • Supervise Care Managers to ensure delivery of comprehensive, integrated and person centered care management services within the communities they serve.
  • Ensure that Care Managers are adhering to policies per funder requirements.
  • Attend webinars, trainings, etc. in order to stay updated and communicate policy/procedure updates to staff.
  • Act as a liaison between Hillside and referring sources.
  • Provide documented, ongoing supervision to staff both individually and as a team.
  • Communicate on behalf of the team with families, adults, internal and external resources and funders.
  • Monitor documentation and monthly billing requirements of all Care Managers and ensure compliance with funder and agency requirements.
  • Provide ongoing training, coaching and mentoring to Care Managers.
  • May attend community meetings on behalf of program.
  • May carry an assigned caseload.
