Staff Software Engineer, Billing & Payments

Carrum Health•,
•$210,000 - $250,000•Remote

About The Position

As a Staff Software Engineer on Carrum Health’s billing and payments platform, you will set the technical direction for systems that price, bill, and pay for each episode of care. The scope includes bundled pricing and contracts, invoicing and provider payments, and financial data used by Finance and Operations. Pricing and billing logic currently spans several services, slowing changes and making financial correctness harder to guarantee. A key initial effort is to centralize pricing in a single pricing service, establish clear service boundaries and billing data models, and create a foundation that supports new service lines without a rewrite. The role works primarily with Ruby on Rails in a Service-Oriented Architecture, React, and PostgreSQL. You will partner with Product, Finance, Operations, and Data, and serve as a technical resource for problems that cross teams or have unclear ownership. Carrum is remote-first; occasional travel for team offsites, client events, or company gatherings may be part of the role.

Requirements

  • 8+ years building and running production systems, with outcomes demonstrating impact.
  • Experience leading technical initiatives across multiple teams in ambiguous problem spaces, from problem framing through measurable results.
  • Experience designing APIs and data models for multiple consumers and evolving them without breaking dependent users.
  • Production experience with Ruby on Rails in distributed architecture, React, PostgreSQL, Docker, and a major cloud platform; Carrum currently runs on AWS.
  • Treat financial correctness, auditability, monitoring, and safe rollback as core requirements.
  • Ability to make tradeoffs explicit, distinguish essential from accidental complexity, and judge when a solution is sufficient for the current need.
  • Ability to explain complex systems clearly to technical and non-technical stakeholders and build trust through follow-through.
  • Daily use of AI tools for design, development, debugging, and documentation, with accountability for submitted work.
  • Entrepreneurial, resourceful approach and ability to make an impact quickly with limited direction.

Nice To Haves

  • Experience with billing, invoicing, payments, or other financial systems.
  • Healthcare payments experience, including bundled or value-based pricing, claims, or provider reimbursement.

Responsibilities

  • Set the technical vision for billing and payments and translate it into an incremental delivery plan.
  • Centralize pricing into a single source of truth and decouple billing from systems it is currently entangled with.
  • Own revenue-critical systems end to end, ensuring they are correct, observable, and safe to change, and detecting problems proactively.
  • Partner with Product and Finance to shape the roadmap, question requirements early, and explain engineering tradeoffs in business terms.
  • Lead technical initiatives spanning multiple teams without relying on positional authority.
  • Write design documents, ADRs, and diagrams that serve as references for how billing works.
  • Mentor and support engineers through design reviews and code reviews, improving technical decisions.
  • Determine where AI can be applied effectively in a correctness-sensitive domain and establish guardrails where appropriate.

Benefits

  • Stock option plan and performance bonus for eligible employees.
  • Flexible schedules and remote work.
  • Flexible PTO, within reason.
  • Summer Fridays, with sign-off at 1 p.m. local time.
  • 15+ paid holidays, including Juneteenth, World Mental Health Day, and Carrum Health Day.
  • Annual holiday shutdown in December.
  • Paid parental leave.
  • Health, vision, and dental insurance.
  • Employee Assistance Programs (EAP).
  • 401K retirement plan.
  • Occasional opportunities to connect virtually and in person.
  • Chicago and San Francisco offices available as an option.
