Staff Product Manager, Care Coordination

Imagine Pediatrics•United States (Remote), TX
•$185,000 - $215,000

About The Position

Imagine Pediatrics is hiring a Staff Product Manager to own the product systems that help care teams capture, assign, track, and resolve family needs. The product area includes shared patient and work views, tasking and handoffs, family messaging by chat, text, and email, internal care team communication, and follow-up after hospital discharges. Primary users include navigators, nurses, resource specialists, pharmacists, and other care team members; families also interact with the product through messages and updates. The role will set product direction for care coordination and determine where AI and automation can reduce coordination work while preserving appropriate human judgment. The position owns the full product lifecycle, from discovery and strategy through delivery, adoption, and measurement. Success is measured by the percentage of family needs resolved within target time, speed to named ownership, stalled needs, and care team coordination overhead. The first-year aims are to make every family need visible with a named owner and status; provide care teams with a shared, prioritized view of patients and work; and put AI-assisted capture, routing, or prioritization into production with measured time savings and no decline in quality. The role also influences the roadmaps of teams whose products connect to care coordination, including outreach and the family app, and mentors other product managers.

Requirements

  • 8+ years of product management experience, ideally in healthcare or care delivery.
  • Track record of owning a workflow-heavy product area from strategy through launch and adoption.
  • Experience shipping AI or automation features for operational users with measured impact on efficiency or outcomes.
  • Sound judgment about AI limitations, including evaluation, failure modes, and when a human must remain in the loop.
  • Experience building tasking, case management, worklist, or messaging systems.
  • Experience redesigning operational workflows for scale rather than only building tools on top of existing processes.
  • Examples of pushing back on senior stakeholders and maintaining boundaries to protect outcomes.
  • Experience leading migrations from legacy systems without disrupting dependent teams.
  • Strong data-modeling instincts and ability to reason about entities, ownership, and state with engineers.
  • Comfort working in a fast-moving startup environment where priorities evolve.
  • Experience mentoring product managers.
  • Excellent communication with technical and non-technical audiences, including executives.
  • Motivation to improve the health and lives of children with special health care needs and their families.
  • Comfort with ambiguity, sound judgment, and ability to bring clarity and momentum to difficult problems.

Nice To Haves

  • Experience with care management platforms or EHR-integrated workflows.
  • Experience with Medicaid populations or value-based care.
  • Experience with LLM-based products in a regulated or clinical setting.

Responsibilities

  • Define multi-quarter strategy for capturing, owning, working, and resolving family needs across care teams.
  • Synthesize input from clinical leaders, care teams, operations, and data to frame problems and make decisions amid ambiguity.
  • Challenge and redesign operational workflows before digitizing them; simplify or automate steps where appropriate.
  • Decline requests that do not advance outcomes, explaining decisions and maintaining boundaries with senior stakeholders.
  • Identify opportunities for AI and automation while preserving human judgment where needed.
  • Partner with engineering and data teams to move AI features from prototype to production with evaluation criteria, safety guardrails, and care team trust.
  • Measure AI and product outcomes, including time saved, faster need resolution, and fewer dropped handoffs.
  • Own the product lifecycle using a build–measure–learn approach, including direct discovery with care teams.
  • Sequence migrations from legacy tools, make scope tradeoffs, and deliver improvements that build on one another.
  • Partner with engineering, clinical leaders, care teams, data, and operations to design workable care coordination workflows.
  • Broker tradeoffs between product, clinical workflows, and operational capacity with senior stakeholders.
  • Define how other teams connect to care coordination and influence their roadmaps where they depend on the product.
  • Own care coordination success metrics for family outcomes and care team efficiency, and hold the organization accountable to them.
  • Drive care team adoption and discontinue or rework product changes that do not improve outcomes.
  • Mentor other product managers and model strong discovery, writing, prioritization, and decision-making.
  • Develop internal expertise in pediatric care coordination and AI-enabled care operations.
