About The Position

This position provides comprehensive, patient-centered care coordination across the health care continuum. The case manager supports safe discharge planning, continuity of care, and coordination of post-acute services, empowering patients and families to participate in the discharge process. Care coordination is tailored to patient needs and payer requirements and accounts for the needs of patients of all ages, including those who are physically disabled, mentally ill, chronically ill, or terminally ill. The role collaborates with patients, families, physicians, payers, and other members of the health care team to develop and communicate discharge plans, address barriers to effective care, and meet utilization and regulatory requirements. It also contributes leadership to process improvement and interdisciplinary care planning, and educates team members about case management and managed care concepts. The position may supervise other staff and has discretion to perform its functions within established procedures, consulting a supervisor about unusual situations. The role is based at Banner-University Medical Center Tucson. The schedule is full time, 40 hours per week, Monday through Friday, with five eight-hour shifts and a stated shift window of 7:00 a.m. to 4:30 p.m. Weekend and holiday rotations are required. The posting describes a four-day work week in its introductory section, but lists five eight-hour shifts in the schedule details. A comprehensive six-week orientation with experienced preceptors is provided. The role uses Windows 11, Cerner, CareAware, and Microsoft Teams; prior experience with these platforms is not required.
