About The Position

BAYADA Home Health Care is hiring an experienced Speech Language Pathologist for a per diem opportunity providing home health visits to adult and geriatric patients on a per-visit basis. The role is part of a multidisciplinary health care team that provides rehabilitative care and skilled nursing to clients so they can receive needed medical care at home. Prior home health experience is not required, but at least one year of experience as a Speech Language Pathologist is required.

Requirements

• At least one year of experience as a Speech Language Pathologist.
• Current Speech Language Pathologist license in Nevada.
• Must hold a Certificate of Clinical Competence in Speech from the American Speech-Language-Hearing Association, or meet the equivalent education and work requirements and be awaiting the certification examination, or meet the required work and education requirements and be accumulating supervised work experience for certification.
• Solid computer skills.

Nice To Haves

• Prior experience with electronic medical records (EMR) is preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Evaluate clients to assess and diagnose speech, language, communication, or swallowing difficulties and provide therapeutic interventions.
• Develop treatment plans based on evaluations, physician orders, and recommended goals.
• Continually assess and revise speech therapy care plans, and participate in multidisciplinary care planning with BAYADA clinicians as appropriate.
• Educate and instruct patients, family members, or other representatives in rehabilitative care and activities that promote health, safety, and independent living.
• Document observations, interventions, evaluations, and services provided using a touch pad tablet.
