About The Position

BAYADA Home Health Care is seeking an experienced Speech Language Pathologist for a per diem position providing home health visits to adult and geriatric patients in homes throughout Stanly County, North Carolina. Prior home care experience is a plus but is not required. The Speech Language Pathologist is an integral member of a multidisciplinary health care team providing rehabilitative care and skilled nursing so clients can receive the medical care they need to remain at home.

Requirements

• Hold a current license as a Speech Language Pathologist in North Carolina.
• Possess a Certificate of Clinical Competence in Speech from the American Speech-Language-Hearing Association, or meet the equivalent educational requirements and work experience and be awaiting the opportunity to take the certification examination, or have completed the necessary work and education requirements and be accumulating the supervised work experience required for certification.
• Have solid computer skills.

Nice To Haves

• Prior home care experience as a Speech Language Pathologist.
• Prior experience with electronic medical records (EMR).

Responsibilities

• Make home visits to clients in designated geographic territories.
• Evaluate clients and assess, diagnose, and provide therapeutic interventions for speech, language, communication, or swallowing difficulties.
• Develop treatment plans based on evaluations, physician orders, and recommended goals.
• Continually assess and revise speech therapy care plans and participate in multidisciplinary care planning with BAYADA clinicians, as appropriate.
• Educate and instruct patients, family members, or other patient representatives in rehabilitative care and activities that promote health, safety, and independent living.
• Document observations, interventions, evaluations, patient care management, and services provided using a touch pad tablet.
