About The Position

Transition Home Healthcare is a home health agency serving Houston, Texas, and surrounding areas. The Speech Language Pathologist facilitates rehabilitation for patients with emotional, physical, or mental disabilities; implements speech therapy care plans; coordinates care with the broader team; and educates patients and family members. The role also requires preparing timely, complete, and accurate clinical notes and communicating effectively with patients, caregivers, healthcare providers, office support, and other care team members.

This is a PRN position with openings in the North, Northeast, and Northwest service areas. Home health experience is highly preferred. The employer encourages bilingual candidates fluent in Spanish to apply. Requirements

• Be a speech language pathologist in the State of Texas.
• Have knowledge of home health regulations and general healthcare compliance.
• Be able to handle a physical workload.
• Have a good driving record.
• Have proficient experience with Microsoft Office, including Word, Excel, PowerPoint, and Outlook.
• Have exceptional communication and people skills.

Nice To Haves

• Home health experience is highly preferred.
• Bilingual fluency in Spanish is encouraged.
• Experience with Home Care Home Base EMR is preferred but not required.
