About The Position

Caris Healthcare Social Workers (LMSW) support the hospice interdisciplinary team through office functions. The role focuses on completing comprehensive patient assessments, identifying care-planning goals, and providing psychosocial and emotional support to patients and caregivers at the end of life. The position includes visits with patients and caregivers in their residential settings and participation in an on-call rotation for after-hours visits when needed, such as for a patient’s death, funeral, or admission.

Requirements

One year of social work experience in a health care setting.
Excellent communication and interpersonal skills.
Tennessee and Missouri: BSW required; MSW preferred.
Virginia: BSW required; MSW preferred.
South Carolina: Social work licensure required; LMSW required.
Georgia: Social work licensure required; LBSW required; LMSW preferred.

Nice To Haves

MSW preferred in Tennessee, Missouri, and Virginia.
LMSW preferred in Georgia.

Responsibilities

Complete and document comprehensive psychosocial assessments with patients in the electronic medical record.
Identify care-planning goals and develop interventions to promote the best outcomes.
Participate in Interdisciplinary Team meetings and provide insight into patients’ psychosocial and emotional needs.
Provide education related to funeral planning and advance directives as needed.
Conduct visits with patients and caregivers in their residential settings.
Assist with bereavement support and interventions for caregivers following a patient’s death.
Participate in an on-call rotation to make after-hours visits when needed for a patient’s death, funeral, admission, or other needs.
