About The Position

A Licensed Master Social Worker (LMSW) in hospice care provides emotional, social, and practical support to terminally ill patients and their families as part of an interdisciplinary healthcare team. The primary function is to provide coordinated care in patients’ homes to patients of all age groups and perform psychosocial assessments, analysis, counseling, and referrals to meet the needs of patients and families.

Requirements

• Bachelor’s degree in Social Work from an accredited school.
• One or more years of hospice experience.
• Valid driver’s license.
• Valid auto insurance.
• AHA CPR certification.

Responsibilities

• Complete the Comprehensive Assessment within five days of the patient’s election of hospice care to identify the patient’s and family’s psychosocial, financial, environmental, and community needs, as evidenced by plan-of-care documentation, clinical records, interdisciplinary group meetings, and community resource referrals.
• Assist with advance directives, funeral planning, discharge planning, and family counseling, and may assist with changes in the level of care as indicated by the patient’s or family’s needs.
• Use effective interpersonal relations and communication skills to achieve desirable outcomes.
• Provide patient and family or caregiver counseling around end-of-life, death, and grief.
• Attend interdisciplinary team meetings, participate in patient care planning, and coordinate and collaborate with the interdisciplinary group to promote coordination of patient care.
• Provide an environment that promotes respect for patients and their privacy and property.
