About The Position

This full-time position with the Perinatal Health Partners Program is located in the Southwest Public Health District/Albany, Georgia. The role involves making a difference in the community with most evenings and weekends off. The Social Worker will provide services to eligible pregnant and postpartum women within a 14-County District, offering psychosocial support, education, and resource referrals. The position requires documentation in the medical record, case management, coordination with providers, and collaboration with community agencies. Responsibilities may also include participation in health fairs, committee work, presentations, and responding to public health emergencies, all while maintaining outstanding customer service and HIPAA standards.

Requirements

  • Master's degree in a related field from an accredited college or university AND eligibility to be licensed OR Bachelor's degree in a related field from an accredited college or university AND Two years of experience in social service delivery.
  • Ability to lift up to 45 lbs.
  • Ability to bend and reach up.
  • Valid driver’s license required.
  • Consent to a motor vehicle record assessment.
  • Male applicants between 18 and 26 years of age must present proof of Selective Service Registration.

Nice To Haves

  • Previous employment with any state’s Department of Public Health or a County Board of Health
  • Experience conducting home visits and patient home assessments
  • Experience with Case Management and Community Outreach
  • Bilingual (Spanish and English).

Responsibilities

  • Participates in home visiting and remote services to eligible pregnant and postpartum women within the 14-County District.
  • Under supervision, makes assessments and provides psychosocial support and education to women and families.
  • Makes resource referrals as necessary.
  • Documents in the medical record/electronic medical record.
  • Provides case management-type services, coordinates with providers of services, and works collaboratively with community agencies and providers.
  • May participate in health fairs, serve on committees, make presentations, and respond to Public Health Emergencies.
  • Provides outstanding customer service and maintains HIPAA Privacy and Security standards.
  • Responds to Public Health Emergencies.
