About The Position

Baptist Health is seeking a master's-level Social Worker II to join the Social Services team at its Baptist Downtown campus. This PRN days opportunity is part of an interdisciplinary care team focused on enhancing well-being through psychosocial support and connection to appropriate resources. The role helps alleviate barriers and reduce the burden of medical conditions by addressing needs that affect patients’ lives.

Requirements

• Master of Social Work degree.
• 1–2 years of social work experience.
• 3–5 years of discharge planning experience.
• Home health experience.

Responsibilities

• Assess psychosocial needs and provide services that enhance patient functioning, emotional well-being, and continued access to community services and resources.
• Provide discharge planning and arrange home health and durable medical equipment (DME).
• Provide counseling and assist with placement and transfers.
• Refer patients to community services.
