About The Position

Wolfson Children’s Hospital, part of Baptist Health, is seeking a Social Worker to join its Behavioral Health team at the Downtown - Main Building. The Social Worker works as part of an interdisciplinary care team to enhance well-being by helping meet needs that affect a person's life through psychosocial support and connection to appropriate resources. The role focuses on alleviating barriers and lessening the burden of medical conditions.

Requirements

• 1–2 years of social work experience.
• 1–2 years of case management experience.
• 1–2 years of healthcare experience.
• Bachelor of Social Work.

Responsibilities

• Assess psychosocial needs and provide services that enhance patient functioning, emotional well-being, and continued access to community services and resources.
• Provide care coordination.
• Provide psychoeducation.
• Provide referrals to community services.
