About The Position

Baptist Medical Center is seeking a Licensed Clinical Social Worker for its Social Services Department at Baptist Jacksonville in Jacksonville, Florida. This is a full-time, day-shift opportunity. The role supports patients’ psychosocial needs and emotional well-being, helps maintain access to community services and resources, and assists with discharge planning and care coordination. A valid Florida LCSW license is required; registered interns will not be considered.

Requirements

• Five years of social work experience.
• Case management experience.
• Master’s degree (MA/MS).
• Valid Licensed Clinical Social Worker (LCSW) license from the Florida Department of Health; registered interns will not be considered.

Responsibilities

• Assess psychosocial needs and provide services that enhance patient functioning, emotional well-being, and continued access to services and community resources.
• Provide discharge planning and care coordination, including arranging home health services and durable medical equipment (DME).
• Provide counseling.
• Assist with placement and transfers.
• Provide referrals to community services.
