About The Position

Baptist Medical Center is seeking a Licensed Clinical Social Worker for its Social Services Department at Baptist Jacksonville in Jacksonville, Florida. This PRN role provides high-level screening, assessment, and care planning for an identified high-risk patient population in the Emergency Department. The position works collaboratively with medical staff and other personnel to coordinate individualized, high-quality, cost-effective care, support safe and appropriate admissions and discharges, and improve patient outcomes and throughput.

A valid Florida Licensed Clinical Social Worker (LCSW) credential is required. Registered interns will not be considered.

Requirements

• Master's degree (MA/MS).
• Valid Florida Licensed Clinical Social Worker (LCSW) credential from the Florida Department of Health; registered interns will not be considered.
• Social work experience.
• Case management experience.
• 3–5 years of discharge planning experience.
• 3–5 years of case management experience.

Responsibilities

• Screen, assess, and plan care for an identified high-risk patient population in the Emergency Department.
• Collaborate with attending and resident physicians, nursing staff, and ancillary, professional, and technical personnel to coordinate high-quality, individualized, cost-effective patient care.
• Use Utilization Review criteria, admission guidelines, and discharge criteria to determine the appropriateness of patient admissions and discharges.
• Implement interventions to enhance patient safety, quality of care, and clinical outcomes.
• Maintain ongoing communication with patients and families, medical staff, caregivers, and third-party payors to establish priorities, coordinate services, and develop and implement goal-directed care plans.
• Promote efficient patient throughput and help reduce inappropriate hospital readmissions.
• Participate in oversight of resource utilization in critical care and emergency services areas to support efficient, cost-effective care delivery.
