About The Position

The Social Services Designee assists residents and their families in dealing with the impact of illness and extended-care placement. The role participates in resident care management and helps ensure residents’ needs are met through social service programming and appropriate referrals to outside agencies and community resources. This is a full-time, Monday-through-Friday position with no weekends or holidays.

Requirements

Bachelor’s degree in a related human services field, such as Social Work, Sociology, Special Education, Rehabilitation Counseling, or Psychology.
At least one year of social work experience in a health care setting working directly with individuals.
Excellent computer skills.
Ability to multitask and work in a fast-paced environment with little supervision.
Working knowledge of basic services provided by skilled nursing and related facilities.
Excellent customer service skills and a commitment to delivering excellent, passionate, innovative care.
Ability to work as a team player and support team members.

Nice To Haves

Licensed Social Worker (LSW) preferred, but not required.
Long-term care experience preferred.
Experience with electronic charting is a plus.

Responsibilities

Assist residents and their families with psychosocial needs related to illness, disability, treatment, or their stay in the facility.
Coordinate services with other facility disciplines to help residents and families promote their well-being.
Maintain a working knowledge of residents’ rights.
Document social service interventions and assessments in resident charts in a timely manner.
Participate in resident care management, including resident care plan conferences and development of multidisciplinary plans of care.
Inform residents and/or responsible parties about roommate and room changes.
Assist with admissions, discharges, and MDS testing.
