About The Position

The Social Services Designee assists the Social Services Director with social service programs and supports residents’ emotional, psychosocial, and social needs. The role provides service coordination and support to residents and families, participates in care planning and interdisciplinary team meetings as assigned, and maintains accurate documentation in accordance with regulatory requirements. The position is based at Frederic Nursing & Rehabilitation Community in Frederic, Wisconsin.

Requirements

Strong written and verbal communication skills and the ability to interact professionally with residents, families, and interdisciplinary teams.
Ability to assist with documentation, assessments, care planning, and regulatory requirements under the direction of a licensed Social Services Director.
Compassionate, resident-focused approach with strong interpersonal skills.
Organized and detail-oriented, with the ability to manage multiple priorities.
Positive, can-do attitude with excellent attendance.

Nice To Haves

Associate’s degree in Human Services, Social Work, Psychology, or a related field; experience may be considered in lieu of a degree.
Experience working in a skilled nursing or long-term care setting.
Working knowledge of resident rights, discharge planning, and psychosocial support services.

Responsibilities

Assist the Social Services Director in implementing and carrying out social service programs in accordance with state and federal guidelines.
Support the identification and documentation of residents’ social, emotional, and psychological needs.
Coordinate services and support for residents and families, including communication and follow-up.
Participate in care planning and interdisciplinary team meetings as assigned.
Maintain accurate and timely documentation to support compliance and regulatory standards.
