About The Position

The Social Services Designee assists residents and their families with adjusting to placement, illness, and the plan of care. Under the direction of the Social Services Director, the designee helps residents attain the highest practicable level of functioning in accordance with applicable federal and state regulations and the organization's policies and procedures. The role includes helping residents and families express treatment wishes and supporting their transition to and adjustment to the facility. The designee also participates in resident and family council meetings as requested, supports Admissions, and assists with discharge planning services.

Requirements

High school diploma or equivalent.
Ability to work with physically and mentally ill, disabled, and emotionally upset elderly individuals.
Ability to organize, prioritize, and meet deadlines.
Ability to maintain confidentiality and discretion.
Knowledge of nursing practices, procedures, terminology, laws, regulations, and guidelines pertaining to long-term care.

Nice To Haves

Experience with the geriatric population.
Associate or bachelor's degree in a related field.
Creative problem-solving skills.
Strong communication skills.
Sufficient computer skills to manage records and documentation.
