About The Position

Oak Crest is seeking a compassionate, organized, and people-focused Social Services Coordinator to support older adults and their families. As part of the interdisciplinary care team, the coordinator builds relationships with residents and families, advocates for residents’ needs and preferences, and helps them navigate transitions. The role includes assessing social and emotional needs, providing guidance and support, coordinating care plans and transitions, connecting residents with services, and maintaining documentation and regulatory compliance. The position starts at $24.15 per hour.

Requirements

• 1–2 years of experience in a healthcare or medical setting; long-term care experience is preferred.
• Strong communication, interpersonal, and organizational skills.
• Ability to collaborate effectively with an interdisciplinary team.
• Compassion, empathy, and a genuine commitment to the dignity and well-being of older adults.

Nice To Haves

• Bachelor’s degree in Social Work or a related field is preferred but not required.
• Experience in long-term care.

Responsibilities

• Advocate for residents’ rights, preferences, and individual needs, ensuring their voices are heard in care decisions.
• Build relationships with residents and families, assess social and emotional needs, and provide ongoing guidance, counseling, and support.
• Coordinate admissions, discharges, internal transfers, and transitions to alternative care settings.
• Collaborate with nursing, therapy, and other team members to develop and coordinate individualized care plans.
• Connect residents with needed services by serving as a liaison among residents, families, healthcare professionals, and community organizations.
• Provide emotional support, problem-solving assistance, and crisis intervention to residents and families.
• Complete required assessments, maintain accurate documentation, monitor care plans, and help ensure compliance with applicable regulations.
