About The Position

Creston Specialty Care, a 74-bed long-term care skilled nursing community in Creston, Iowa, is hiring a Social Services Coordinator. The role provides comprehensive social services and support to residents and their families throughout their healthcare journey. The coordinator assesses psychosocial needs, offers counseling and emotional support, assists with adjustment and care transitions, and works with the care team and community resources to promote residents’ quality of life and outcomes.

Requirements

• Bachelor’s degree in social work, gerontology, or a related health and human services field, including sociology, special education, rehabilitation, counseling, or psychology.
• Ability to obtain a Licensed Social Worker (LSW) credential.
• At least one year of supervised social work experience in a healthcare setting, working directly with individuals.
• Interpersonal and communication skills, including the ability to communicate positively and effectively while completing work accurately and on time.
• Strong commitment to helping others and consistently treating them with empathy, respect, patience, and discretion.

Responsibilities

• Assess residents’ psychosocial needs and complete social histories.
• Complete assigned MDS sections accurately and on time.
• Document care plans, progress, and follow-up in accordance with regulations.
• Provide counseling and emotional support to residents and families.
• Support residents and families with adjustment, rights, and orientation.
• Assist with discharge planning and care transitions.
• Coordinate referrals and act as a liaison with community resources.
• Partner with the care team to enhance quality of life and outcomes.
• Plan and support resident, family, and community engagement activities.
• Promote facility programs and services to referral partners.
