About The Position

The Springs at Monarch Landing is seeking a hospitality-focused Social Services Coordinator for a full-time position at The Springs Health Center. The coordinator supports residents and their families through life transitions by helping ensure residents receive emotional, social, and psychological support. The role involves building relationships, assessing needs, coordinating care, assisting with discharge planning, and advocating for residents’ dignity, independence, and quality of life. The position is Monday through Friday, 8:30 a.m. to 5:00 p.m., with a starting salary of $20 per hour. It reports directly to the Social Services Manager.

Requirements

• Bachelor’s degree in Social Services or a related field required.
• Knowledge of federal and state regulations for skilled nursing facilities.
• Strong communication skills and a heart for service.
• Dependable, compassionate, and calm under pressure.

Nice To Haves

• Master’s degree preferred.
• A minimum of three years of social service experience within a skilled nursing facility preferred.
• An equivalent combination of education and direct experience may be considered.

Responsibilities

• Build meaningful relationships with residents and their families.
• Assess residents’ social, emotional, psychological, and adjustment needs.
• Develop and coordinate individualized care plans.
• Provide support and guidance during challenging life transitions.
• Assist residents and families with discharge planning and community resources.
• Support residents experiencing dementia, behavioral health concerns, grief, or loss.
• Collaborate with nursing, therapy, physicians, and interdisciplinary team members to support quality resident outcomes.
• Maintain accurate documentation and ensure compliance with federal, state, and community regulations.
• Advocate for residents while promoting dignity, independence, and quality of life.
