About The Position

Care Initiatives is hiring a Social Services Coordinator to provide comprehensive support to residents and their families throughout their healthcare journey. The role serves residents at one of the organization’s skilled nursing communities in Iowa and contributes to resident well-being and quality of life by assessing psychosocial needs, providing emotional support, supporting care transitions, and coordinating with the care team and community resources.

Requirements

• Bachelor’s degree in social work, gerontology, or a related field within health and human services, including sociology, special education, rehabilitation, counseling, or psychology.
• Ability to obtain a Licensed Social Worker (LSW) credential.
• At least one year of supervised social work experience in a healthcare setting, working directly with individuals.
• Interpersonal and communication skills to communicate positively and effectively while completing work accurately and on time.
• Commitment to helping others and consistently treating them with empathy, respect, patience, and discretion.

Responsibilities

• Assess residents’ psychosocial needs and complete social histories.
• Complete assigned MDS sections accurately and on time.
• Document care plans, progress, and follow-up in accordance with regulations.
• Provide counseling and emotional support to residents and families.
• Support residents and families with adjustment, rights, and orientation.
• Assist with discharge planning and care transitions.
• Coordinate referrals and act as a liaison with community resources.
• Partner with the care team to enhance quality of life and outcomes.
• Plan and support resident, family, and community engagement activities.
• Promote facility programs and services to referral partners.
