About The Position

Dubuque Specialty Care, an 84-bed long-term care skilled nursing community in Dubuque, Iowa, is hiring a Social Services Coordinator. The role provides comprehensive social services to residents and their families throughout the healthcare journey, helping support residents’ well-being, quality of life, and care outcomes. The coordinator works with residents, families, the care team, and community resources, and supports facility programs and community engagement.

Requirements

• Bachelor’s degree in social work, gerontology, or a related health and human services field, including sociology, special education, rehabilitation, counseling, or psychology.
• Ability to obtain a Licensed Social Worker (LSW) credential.
• At least one year of supervised social work experience in a healthcare setting, working directly with individuals.
• Interpersonal and communication skills to communicate positively and effectively while completing work accurately and on time.
• Commitment to helping others and consistently treating them with empathy, respect, patience, and discretion.

Responsibilities

• Assess residents’ psychosocial needs and complete social histories.
• Complete assigned MDS sections accurately and on time.
• Document care plans, progress, and follow-up in accordance with regulations.
• Provide counseling and emotional support to residents and families.
• Support residents and families with adjustment, rights, and orientation.
• Assist with discharge planning and care transitions.
• Coordinate referrals and act as a liaison with community resources.
• Partner with the care team to enhance quality of life and outcomes.
• Plan and support resident, family, and community engagement activities.
• Promote facility programs and services to referral partners.
