About The Position
Brookestone View is seeking a compassionate, relationship-focused Social Services Coordinator to enhance residents’ lives in a senior living and healthcare environment. The coordinator advocates for residents by helping them navigate challenges, access resources, and maintain the highest possible quality of life. Working closely with residents, families, and the care team, the coordinator helps create meaningful experiences and supports individualized care plans that promote dignity, choice, and well-being. The role also supports residents through life transitions and helps foster a warm, welcoming community.
Requirements
