About The Position

The Social Services Assistant supports the Director of Social Services in implementing social care plans and resident assessments in accordance with applicable federal, state, and local standards, guidelines, and regulations. The role helps ensure that each resident’s medically related emotional and social needs are met and maintained. The assistant supports residents and families with care planning, financial assistance resources, documentation, discharge planning, community outreach, education, and regulatory compliance.

Requirements

• High school diploma or GED/HSE preferred.
• 1–3 years of relevant experience preferred.

Responsibilities

• Support residents and families by developing, reviewing, and updating individualized care plans and social service assessments.
• Guide residents toward Medicare, Medicaid, and other financial assistance resources to help them access needed support.
• Document resident progress, services provided, and family interactions clearly and promptly.
• Collaborate with the care team, residents, and families on discharge planning and continuity of care.
• Participate in community outreach and planning to support resident well-being and enhance campus services.
• Educate residents, families, and staff about social service programs, resident rights, and company policies.
• Assist with survey preparation and ensure compliance with state and federal regulations.
• Assist the Director of Social Services in implementing social care plans and resident assessments and meeting residents’ medically related emotional and social needs.
