About The Position

Avalon Care Center Tacoma is seeking a part-time Social Services Assistant to support residents and families through important life transitions. The role focuses on helping residents maintain dignity, independence, and quality of life during their stay, while building relationships and working collaboratively with residents, families, interdisciplinary team members, and community resources. The position is based in Tacoma, Washington, and is scheduled Monday through Friday. Pay is $26.00–$28.00 per hour, based on experience, qualifications, and skills.

Requirements

• Skilled nursing facility or healthcare experience required.
• Strong communication and interpersonal skills.
• Ability to work effectively within a team.
• Basic computer skills.
• Strong organizational and documentation skills.
• Ability to pass employment screenings.

Responsibilities

• Support residents and families during admission, care planning, and discharge processes.
• Help residents adjust to changes and transitions during their stay.
• Assist with psychosocial assessments, care plans, and resident documentation.
• Advocate for resident rights and promote resident well-being.
• Coordinate with interdisciplinary team members to support quality outcomes.
• Build positive relationships with residents, families, and community resources.
