Youth Futures seeks a case manager to serve in a residential youth shelter. A case manager will be responsible for connecting youth to resources, oversee goal setting and wellness tracking, as well as, billing services and maintaining accurate records. The case manager will work one-on-one with clients, handles client problems case-by-case, which might include struggles with domestic violence, physical or emotional illness, alcoholism, drug abuse and other conditions that contributed to their homelessness. This position will coordinate access to services for permanent housing, health care, financial help, family mediation /and or job training. A case manager will drive or coordinate transportation for residents to community services for help with permanent housing, health care, drug or alcohol referral, educational supportive services and assist in access to permanent self-sufficiency. Each of these pillars outlines the essential functions and primary goals of the role of the Case Manager. The Case Manager provides critical guidance for youth in crisis with these primary goals: Providing a Safe Space: ? Crisis response, assisting youth in crisis to enter shelter and meet immediate needs ? Initiating risk assessments and mental health needs to find appropriate resources ? Providing trauma informed care and wrap around services ? Working with staff and therapists to determine needs and supervision Creating Plans for Success: ? Assessing strengths and resources of youth ? Goal planning with youth to follow youth-led priorities ? Innovative problem-solving, identifying relevant resources ? Meeting each youth where they are at, providing encouragement, reminders, arranging transportation and building skills needed for success ? Providing ongoing wellness checks and assessments with ongoing case management Supporting Successful Youth Futures: ? Coordinating Life Skills workshops and one-on-one mentorship to support self-reliance and to foster skills needed for transitions to adulthood ? Supporting education with enrollment, transportation, tutoring assistance, and connecting with liaisons and community partners for academic achievement ? Facilitating health and safety, oversee the medical needs of the youth, including access to healthcare, dental, and vision needs ? Assist youth seeking employment and housing with skills and tools needed for success ? Support engagement and healthy connections with friends, family and community and support recreation and healthy coping ? Facilitating access to mental health resources, counseling and therapy, as well as for family and household, to support housing and reunification goals ? Make referrals to outside resources Facilitating a Return to Safe Housing: ? Exploring safe housing plans, whether a return to parents/guardian, other appropriate kin relations, independent living, or other residential support programs. ? Accessing outside programs such as Job Corps, WIA, WFS, etc. Ongoing Support & Aftercare: ? Working with youth and families after discharging from the shelter to ensure a successful transition to safe housing ? Assisting with food, furnishings, or other items needed to support successful transitions ? Providing referrals, resources, as needed Communicating Progress: ? Providing an adequate documentation to make sure that our efforts and youth progress are reportable to responsible parties, such as licensing agencies, funders and donors, as appropriate ? Ensuring that documentation and communication follow procedures for confidentiality to protect the rights and privacy of clients ? Participating in weekly case management strategy meetings, interagency collaborations, and completing daily reporting and weekly summaries, as well as maintaining records of contacts for future needs ? Collaborate with youth, therapists, staff, families, and community partners as needed to provide wrap around support and to make and achieve appropriate and sustainable housing goals for youth. ? Maintain all necessary paperwork and releases that enable youth to safely and legally participate in activities. ? Other duties as assigned.
