About The Position
Chrysalis Host Home is seeking families to open their homes to an adult with intellectual and developmental disabilities. Similar to foster care, the Host Home program provides a loving and nurturing family home for an adult with disabilities. Host Home Providers work from home while caring for the individual and supporting their needs. The role is suited to family structures that enjoy caring for others and can provide a safe, clean environment, build rapport, offer support, and advocate for the individual. This is a contracted position. Monthly payment is determined by the level of care required and includes a room and board payment plus a daily rate for each day the individual is in the provider’s care.
Requirements
- All adults in the home must pass a criminal background check.
- Must be 21 years of age or older.
- Must have experience providing support to individuals with disabilities.
- Must meet home or apartment safety and health requirements.
- Must complete pre-service training requirements.
- Must be able to communicate well verbally and in writing.
- Must have and maintain a valid driver's license and a clean driving record.
- Must have good judgment.
- Must have a positive attitude.
Nice To Haves
- Experience working with people with disabilities.
- Nursing or CNA background.
- Teacher or aide background.
- Caregiver background.
Responsibilities
- Transport the individual to medical appointments, schedule appointments, and attend them.
- Transport the individual to and from activities as needed.
- Complete monthly paperwork.
- Maintain a safe and clean environment for the individual.
- Assist the individual with daily living skills and individual goals.
