About The Position
Chrysalis Host Home is seeking families to open their homes to an adult with intellectual and developmental disabilities. Similar to foster care, the Host Home program provides a loving, nurturing family environment. Host Home Providers work from home while caring for and supporting an individual with disabilities. The role is suitable for a variety of family structures and requires providing a safe, clean environment, building rapport, and advocating for the individual. This is a contracted position. Monthly payment is determined by the level of care needed and includes a room and board payment plus a daily rate for each day the individual is in the provider’s care.
Requirements
- All adults in the home must pass a criminal background check.
- Must be at least 21 years of age.
- Must have experience providing support to individuals with disabilities.
- Must meet home or apartment safety and health requirements.
- Must complete pre-service training requirements.
- Must be capable of communicating effectively in verbal and written form.
- Must have and maintain a valid driver's license and a clean driving record.
- Must have good judgment.
- Must have a positive attitude.
Nice To Haves
- Experience working with people with disabilities.
- Nursing or CNA background.
- Teacher or aide background.
- Caregiver background.
Responsibilities
- Transport, schedule, and attend medical appointments with the individual.
- Transport the individual to and from activities as needed.
- Complete monthly paperwork.
- Maintain a safe and clean environment for the individual.
- Assist the individual with daily living skills and individual goals.
