About The Position

The Threshold Incorporated is seeking an organized, compassionate, and motivated Service Coordinator to support its Community Programs. The position coordinates meaningful community activities, supports individuals with disabilities, and helps ensure programs run smoothly. The Service Coordinator helps individuals build independence, develop life skills, and connect with their community.

Requirements

• Associate degree and two years of related experience, or an equivalent combination of education and experience.
• Strong organizational, communication, and problem-solving skills.
• Ability to coordinate multiple activities and work collaboratively with others.
• Valid Wisconsin driver's license, good driving record, and access to an adequately insured vehicle.

Responsibilities

• Plan and coordinate community outings, recreational activities, and skill-building opportunities.
• Support individuals in developing independence, social skills, and daily living skills.
• Provide guidance and support to direct service staff.
• Maintain client documentation, progress reports, and individualized support strategies.
• Collaborate with families, staff, and community partners.
