About The Position

The Service Coordinator (Mental Health Case Manager) assesses clients to identify individual problem areas necessitating treatment and completes assessments. The role coordinates interdisciplinary team meetings and discharge planning among the multidisciplinary treatment team. The Service Coordinator coordinates implementation of the client’s overall treatment plan on the unit, working closely with nursing staff to ensure continuity of treatment efforts. Crestwood San Diego provides person-centered behavioral health services supporting clients’ recovery, resilience, wellness, psychiatric stabilization, and inclusion of community and natural supports. The role offers experience and training with a behavioral health provider that provides education and training to support employee career growth.

Requirements

Two years of full-time experience, or part-time equivalent, in a mental health setting; a bachelor’s degree with a major in psychology, social work, or behavioral sciences may substitute for this experience.
Demonstrated knowledge of DSM-5 and psychosocial rehabilitation and recovery principles.
Familiarity with regulatory standards.
