Service Coordinator | Mental Health Case Manager
About The Position
The Service Coordinator (Mental Health Case Manager) assesses clients to identify individual problem areas that necessitate treatment and completes assessments. The role coordinates interdisciplinary team meetings and discharge planning among the multidisciplinary treatment team. The Service Coordinator coordinates implementation of the overall treatment plan for clients on the unit and works closely with nursing staff to ensure continuity of treatment efforts. The position is based at Crestwood’s Sonoma County Healing Center, whose Psychiatric Health Facility program provides structure and support to promote stabilization and recovery. The campus uses collaboration, empowerment, a healing environment, peer providers, family partners, and a recovery-oriented milieu to support clients’ return to their communities.
Requirements
- Two years of full-time experience, or part-time equivalent, in a mental health setting; a bachelor’s degree with a major in psychology, social work, or behavioral sciences may substitute for this experience.
- Demonstrated knowledge of DSM-5 and psychosocial rehabilitation and recovery principles.
- Familiarity with regulatory standards.
