About The Position

The Service Coordinator (Mental Health Case Manager) assesses clients to identify individual problem areas requiring treatment and completes assessments. The role coordinates interdisciplinary team meetings and discharge planning among the multidisciplinary treatment team. The Service Coordinator coordinates implementation of each client’s overall treatment plan on the unit and works closely with nursing staff to ensure continuity of treatment efforts. The position is on-call, with AM and PM schedules, at Crestwood Center, a Mental Health Rehabilitation Center in Angwin, California. Crestwood Center offers classes for clients participating in recovery programs and encourages clients to participate actively in treatment planning from admission.

Requirements

Two years of full-time experience, or part-time equivalent, in a mental health setting; a bachelor’s degree with a major in psychology, social work, or behavioral sciences may substitute for this experience.
Demonstrated knowledge of DSM-5 and psychosocial rehabilitation and recovery principles.
Familiarity with regulatory standards.
