About The Position

The Service Coordinator (Mental Health Case Manager) assesses clients to identify individual problem areas that necessitate treatment and completes assessments. The role coordinates interdisciplinary team meetings and discharge planning among the multidisciplinary treatment team. The Service Coordinator coordinates implementation of the client’s overall treatment plan on the unit and works closely with nursing staff to ensure continuity of treatment efforts. The position is based at Crestwood’s San Francisco Healing Center, whose services support clients in recovery through wellness tools and staff support. The program aims to restore hope, empowerment, inclusion of community and natural supports, and a sense of realized potential, as well as to provide psychiatric stabilization.

Requirements

Two years of full-time experience, or part-time equivalent, in a mental health setting; a bachelor’s degree with a major in psychology, social work, or behavioral sciences may be substituted.
Demonstrated knowledge of DSM-5 and psychosocial rehabilitation and recovery principles.
Familiarity with regulatory standards.
