About The Position

As a Supervisor on our Long Term Support and Services team, you’ll lead a dedicated unit that supports individuals in accessing services and long-term support solutions—most often referred to as waivers—through Pennsylvania’s Community HealthChoices program. You’ll guide a team of Service Coordinators as they help individuals navigate complex systems, coordinate care, and promote independence through person-centered planning. This role is ideal for someone who brings strong leadership, empathy, and an understanding of how to manage both people and processes in a high-accountability environment.

Requirements

• RN, or a Pennsylvania-licensed social worker or licensed mental health professional
• At least three (3) years of relevant experience
• A valid Pennsylvania driver’s license, automobile, and required clearances

• Supervisory, leadership, or management experience preferred
• Ability to manage time effectively and respond to changing priorities

Responsibilities

• Ensure that service plans are developed and implemented
• Ensure monitoring and follow-up are conducted appropriately
• Verify waiver eligibility and Medical Assistance requirements
• Support employees in meeting documentation and billing standards
• Monitor unit productivity and provide coaching as needed
• Oversee the incident management and reporting process
• Ensure timely and accurate completion of assessments and planning documents
• Provide direct supervision and caseload support when necessary
• Complete required trainings and prepare program reports
• Maintain cooperative and professional relationships with colleagues and stakeholders
