About The Position

Sprenger Health Care is hiring an RN or LPN MDS for Main Street Care Center in Avon Lake, Ohio. The role coordinates resident assessment processes and related documentation in accordance with state and federal regulations. Responsibilities include scheduling required assessments and care plan meetings, entering assessment data into the electronic charting system, ensuring assessments are completed accurately, and tracking resident admissions, discharges, and re-entries. Sprenger Health Care is a third-generation developer, owner, and operator of senior living communities and services in Ohio and South Carolina. Its services include long-term nursing care, assisted and independent living, memory care, hospice, home care, respite care, and short-term post-acute rehabilitation. Requirements

LPN qualification.
Computer knowledge.
Good organizational and follow-through skills.
Good communication skills.
Ability to be flexible with work hours as needed.

Nice To Haves

Knowledge of MDS.
Experience in long-term care.

Responsibilities

Coordinate the RAI process for assigned residents according to state and federal regulations.
Schedule all OBRA-required and PPS assessments according to state and federal regulations.
Enter information from resident assessments into the electronic charting system.
Ensure accurate completion of all assessments.
Complete and track facility admissions, discharges, and re-entries.
Schedule plan of care meetings for all residents.
