About The Position

The MDS Nurse Coordinator is responsible for the timely and accurate completion of the Resident Assessment Instrument (RAI) process and care management process from admission through discharge. Work must comply with company policies and procedures, Federal, State, and Certification guidelines, and requirements of other applicable entities. Duties include Minimum Data Set completion and admission and discharge tracking. With direction from the VP of Clinical Reimbursement, the coordinator may also coordinate clinical department information systems operations and education.

Requirements

Current, unencumbered, active license to practice as an LPN/LVN or RN in the state.
Training in rehabilitative and restorative nursing practices.
Working knowledge of managed care rules, regulations, and guidelines.
Ability to plan, organize, develop, implement, and interpret programs, goals, objectives, policies, and procedures necessary to provide quality care.

Nice To Haves

Three years of MDS experience in a nursing care facility is preferred.

Responsibilities

Complete the RAI and care management processes accurately and on time from admission to discharge, in accordance with company policies and applicable Federal, State, Certification, and other requirements.
Complete Minimum Data Set work and admission and discharge tracking.
With direction from the VP of Clinical Reimbursement, may coordinate information systems operations and education for the clinical department.
