About The Position
Amedisys, part of the Optum family of businesses, provides home health, hospice, and community-based services. The Hospice RN On-Call provides after-hours clinical support, triages patient needs, and coordinates care to support continuity and quality outcomes. The role combines remote and field-based nursing support, patient assessments, and collaboration with interdisciplinary teams to address urgent and emergent needs.
Requirements
- Current RN license specific to the state(s) where assigned to work.
- At least 1 year of nursing experience.
- Current CPR certification.
- Valid driver's license, reliable transportation, and liability insurance.
- Pass a drug test before beginning employment.

- Nursing experience in hospice, home health, or acute care.
Responsibilities
- Provide on-call nursing support, triage patient needs, and determine appropriate interventions.
- Assess patient conditions by phone and/or in-person visits, escalating care as needed.
- Coordinate with physicians, care teams, and caregivers to support timely and effective care delivery.
- Educate patients and caregivers about symptom management and care plans.
- Maintain accurate and timely documentation of patient interactions and clinical decisions.
