About The Position

KeystoneCare is a nonprofit provider of in-home healthcare services, including home health, hospice, palliative medicine, and private-duty care across the Greater Philadelphia region. The Hospice On-Call RN provides responsive clinical support to patients and caregivers after regular hours, using skilled nursing judgment to address urgent situations and promote comfort and support. The nurse coordinates closely with the broader care team and community providers.

Requirements

• Nursing degree required.
• 1–2 years of hospice or palliative care nursing experience.
• Current unrestricted RN license in Pennsylvania.
• Valid driver’s license and reliable vehicle for on-call patient visits.
• Eligibility to work in the United States.
• Availability for Monday through Thursday and Friday evening on-call coverage.
• Availability for Friday evening through Monday morning on-call coverage.

Nice To Haves

• Active CPR certification.

Responsibilities

• Provide on-call hospice support to patients and families after regular hours.
• Assess urgent symptoms and coordinate timely interventions with the interdisciplinary care team.
• Respond to patient and caregiver calls with compassionate clinical guidance.
• Document on-call assessments, interventions, and outcomes in the electronic medical record.
• Coordinate emergency medication, equipment, and service needs with community providers.
