About The Position

The Hospice RN On-Call responds from home to after-hours requests from the Triage RN, addressing them by telephone or through in-person visits. Visits may include skilled assessments and interventions, after-hours admissions, and end-of-life care. The role performs nursing assessments, coordinates and revises patient care plans, maintains continuity of care, communicates patient progress, documents care, educates families, and makes referrals as needed. The schedule includes every other weekend and alternating weeknights. Compensation is $83,200–$86,000 salaried, plus mileage. Benefits begin on the first of the month after hire; eligibility may vary by position and shift status, and participation in some benefits requires enrollment in an Amedisys medical plan.

Requirements

• Current RN license specific to the state or states assigned.
• At least one year of nursing experience.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.

Responsibilities

• Respond to after-hours telephone requests from the Triage RN by phone or through in-person visits, including skilled assessments, interventions, after-hours admissions, and end-of-life visits.
• Perform initial nursing assessments for new admissions.
• Complete, coordinate, and revise the total plan of care, maintaining continuity by collaborating with appropriate staff.
• Regularly communicate patient progress to the clinical manager, physician, and care team.
• Develop, prepare, and maintain accurate, timely, individualized patient care progress records according to care center policies.
• Record changes and outcomes in pain and symptom management as appropriate.
• Educate families about the patient's condition, needs, and care.
• Refer patients to other hospice care disciplines as needed, or document the rationale for not doing so.
• Perform assigned on-call responsibilities and services for patients and families.
• Perform other duties as assigned.
