About The Position

James River Hospice is seeking a compassionate and committed Hospice RN to provide intermittent hospice case management for assigned patients in their places of residence. The nurse coordinates with the interdisciplinary team to support an optimal patient-care experience and serves patients in Lynchburg and surrounding counties. The position requires travel to Lynchburg as needed and follows an on-call schedule from Friday at 5:00 PM through Monday at 8:00 AM.

Requirements

• Current Registered Nurse license to practice in the Commonwealth of Virginia.
• At least one year of hospice experience.
• Graduate of an accredited nursing school.
• Valid driver's license.
• Reliable car available for use during work hours.

Responsibilities

• Complete an initial assessment of the patient and family to determine hospice needs.
• Develop a care plan with goals based on nursing diagnoses and incorporating palliative nursing actions.
• Coordinate with the clinical team supporting the patient and supervise LPNs and hospice aides involved in patient support.
• Spend time with patients to assist with pain management and provide support.
• Modify and reevaluate the plan of care when necessary.
• Monitor, record, and report symptoms and changes in the patient's condition.
• Obtain and receive physician orders as required.
• Participate in on-call duties as defined by the on-call policy.
• Perform other duties as assigned.
