About The Position

Elara Caring is hiring an After-Hours/On Call Registered Nurse to provide hospice care to patients and support their families. The nurse assesses patient and family needs, monitors patient condition, communicates important changes to the physician and care team, and helps patients, families, caregivers, and facility staff follow the interdisciplinary Plan of Care. The role also includes education on hospice philosophy, pain management, and symptom management across care settings, along with compassionate support at the time of death and bereavement assessments for families and caregivers.

Requirements

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associates, or Baccalaureate School of Nursing.
• At least 1 year of experience as a nurse in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Must possess a valid driver's license and maintain an acceptable driving record, or have reliable transportation to travel between patient homes and assigned work locations.

Nice To Haves

• Home Health experience is preferred, but not required.
• Experience in a hospice or home health environment is preferred.

Responsibilities

• Assess hospice patients and families to identify physical, psychological, social, spiritual, and environmental needs.
• Monitor patient condition and communicate significant changes, concerns, or safety issues to the physician and care team.
• Educate and support patients, families, caregivers, and facility staff in following the interdisciplinary Plan of Care.
• Provide education on hospice philosophy, pain management, and symptom management to staff in nursing homes, hospitals, assisted living facilities, and other care settings.
• Provide compassionate support at the time of death and complete bereavement assessments for families and caregivers.
