About The Position

KeystoneCare is a nonprofit provider of in-home healthcare services across the Greater Philadelphia region, offering home health, hospice, palliative medicine, and private-duty care. The Hospice On-Call RN provides responsive clinical support to hospice patients and caregivers after regular hours. The role uses skilled nursing judgment to address urgent situations, promote patient comfort, and coordinate care with the broader team.

Requirements

• Nursing degree required.
• 1–2 years of hospice or palliative care nursing experience.
• Current unrestricted RN license in Pennsylvania.
• Valid driver’s license and reliable vehicle for on-call patient visits.
• Eligibility to work in the United States.
• Availability for the specified on-call coverage periods.

Nice To Haves

• Active CPR certification.

Responsibilities

• Provide on-call hospice support to patients and families after regular hours.
• Assess urgent symptoms and coordinate timely interventions with the interdisciplinary care team.
• Respond to patient and caregiver calls with compassionate clinical guidance.
• Document on-call assessments, interventions, and outcomes in the electronic medical record.
• Coordinate emergency medication, equipment, and service needs with community providers.
• Provide on-call coverage Monday through Thursday and Friday evening, and Friday evening through Monday morning.
