About The Position

Amedisys, part of the Optum family of businesses, provides home health, hospice, and community-based services. The Hospice RN On-Call provides after-hours clinical support, triages patient needs, and coordinates care to promote continuity and quality outcomes. The role includes remote and field-based nursing support, assessing patient conditions, and collaborating with interdisciplinary teams to address urgent and emergent needs. The position offers a $5,000 sign-on bonus for external candidates. The annual salary range is $71,640 to $107,460 based on full-time employment. Pay depends on factors including local labor markets, education, work experience, and certifications. The role includes a comprehensive benefits package and incentive and recognition programs, as well as equity stock purchase and 401(k) contribution, subject to eligibility requirements. Candidates must pass a drug test before beginning employment.

Requirements

Current RN license specific to the state(s) assigned.
At least 1 year of nursing experience.
Current CPR certification.
Valid driver's license, reliable transportation, and liability insurance.
Pass a drug test before beginning employment.

Responsibilities

Provide on-call nursing support, triaging patient needs and determining appropriate interventions.
Assess patient conditions by phone and/or through in-person visits and escalate care as needed.
Coordinate with physicians, care teams, and caregivers to support timely and effective care delivery.
Educate patients and caregivers about symptom management and care plans.
Maintain accurate and timely documentation of patient interactions and clinical decisions.
